​​passing grade in (5) classes​ 
please bring your most current report card to clinic days​

ALL students NEW TO EMS:
MUST​​ bring the required teacher recommendation form to clinic days​

​​​​Please Be Prepared​​:
have a nutritious meal/snack before coming each day
​wear comfortable clothes/shoes​​​
hair should be pulled up ​ 
bring plenty of water​
